Please refer to the HTML code (or text file, they are identical) and the JavaScript function "function prepopulate()" currently the data is being pulled from an internal command to populate the array "arrayValue" . A new database was created "ppa_panel_test" with the table "ppa_panelists" New code is required, that uses the inputted value "id" or "email" to pull the SQL record and populate the "arrayValue" Also this function (or new function) is required to write back an updated field in this record, the field name is "src" and is updated with a "1" value.
